Astronaut safety should be priority

I have been mulling over the recent events with the Columbia disaster, and I find it very troubling that more was not done to try to prevent or reduce the risk.
It seems to me that after in excess of 100 missions that more has not been done to allow rescue capabilities. I am aware that these missions are on the edge of technology, and of course, I am not a space engineer.
